Confronting Physician- Assisted Suicide and Euthanasia: My Father’s Death” by Susan Wolf

In the article â€Å"Confronting Physician- Assisted Suicide and Euthanasia: My Father’s death† by Susan Wolf, I would also be â€Å"forced to rethink my objections to legalizing assisted suicide and euthanasia†. (Wolf, 2008) I have never been put in this type of a situation where someone’s life depended on it. Having to make the decision to help someone move toward death sooner to ease the pain and suffering they are dealing with, is not something that can be taken lightly. Being left to make the final decision on ending someone’s life to help end their suffering can be completely hard to imagine doing. Watching someone suffer so much can also be very hard to endure. I have never been put in this type of situation and I hope I never have to but I am going to put myself in Susan Wolf’s place and give my point of view of the situation she was put in on such a big decision of a loved one. There are a lot of people who would rather end their life than continue to go on living with the pain and suffering they are dealing with. About three or four years ago, my father in law was suffering from Pulmonary Fibrosis, a lung condition that the tissues deep in the lungs become thick or scarred over time and some people would describe the lungs looking like honeycombs. He was diagnosed in 2000 and was only given about three years to live but lived for six. There were times he would say he â€Å"wishes there was a switch to turn off because he cannot take anymore of the suffering. † He was ready to end his life and the suffering but he kept going until November 15, 2008 when he passed away. He made sure that there was a do not resuscitate in place because he did not want to continue to suffer. Susan’s father’s physical health was declining and it was affecting his mental capacity as well.. She describes him as â€Å" a smart, savvy lawyer, the family patriarch. She could see his spark for life start to fade at the end when he could not even read or do the New York Times crossword puzzles he used to knock off in an hour, or even watch T. V. (Wolf, 2008) I would wonder how much worse this would become and how much longer he would have to live with the pain and the suffering of it all. It is not something I would want to endure myself. Susan’s father was ready to accelerate his departure and she wanted to consider the options and let him know his options as well. I would have done the same thing. Discuss the options available to make sure he gets what he wants. She was watching her father’s health dwindle so fast that she could ot let him continue to suffer anymore. Her father was the man who insisted that â€Å" he would want everything, even in a persistent vegetative state† (Wolf, 2008) was throwing in the towel and wanting to stop any more treatments and to further â€Å"accelerate† (Wolf, 2008) the process, had to be devastating reality that he had to face and something no one wants to face at all. There are a lot of people who think it is o. k. to assist with the ending of one’s life if the situation is right for it. There really is no way to tell if anyone wants to end their suffering unless there is written proof that they want to do so and have it signed and notarized by the patient. Everyone should have a say when it comes to our bodies especially if we are dying and suffering really bad. It will take the people speaking up and having their concerns heard before something can ever take place for people who are suffering very badly and do not want to suffer anymore. What about someone on life support that is in a lot of pain but cannot speak for their self? The decision is left to the families and sometimes the families opt to keep the person living even though that person may be in a world of pain but cannot speak for themselves. Then there are the ones that do take their loved one off of life support to stop the suffering. It is the same thing. Deciding what is right for the dying family member. It is a very tough decision to make just like it was for Susan Wolf in respecting her father and his wishes to just die and end the suffering. I do not think Susan made a wrong decision she help decide for her father. He was able to go naturally which she justified on her own level of moral. Assisted suicide is not a wrong choice if someone can no longer live with the pain and suffering they are enduring. It would be better to have paper work to prove this is what the person wants rather than the person taking their own life. I for one suffer on a daily basis with chronic pain that I cannot get diagnosed. There are days I am in more pain than I ever want to be and just want to end it for good but I am young and have four children and a husband I could not bear to leave behind. I deal with the pain and take medication to help keep it under control. I do hope that one day if it gets out of control, that I have the option to terminate my life if need be. Globalization is an economic activity in which social change is a continuous process. Globalization happens through the increasing level of interdependence among nations to integrate their economic, political and social spheres. It is a force that has sped up the rate of communication, the increased the intensity of market competition, flared the rate of trades and strengthened interaction amongst states. The development of this concept is towards within the world political economy. As Douglas recounted, the concepts that signaled the rise of globalization are: â€Å"the rise of neoliberal transnational technocracy, the transfiguration of the military-industrial complex into the military-communications complex; the rationalization of the postwar ‘global governance institutions (IMF, IBRD, GATT, NATO, UN, EC/U), and the rise of others (NAFTA, WTO, APEC, ASEAN, PECC); transition within the ‘multi-core complex’ (in particular the incorporation of the developmental states of East Asia); decolonization, dependent development and the reconstruction of hierarchical commodity chains against calls for a ‘New International Economic Order’; the disintegration of the corporate-liberal synthesis followed by the deeper embeddedness of state-capital relations; a broad transition of command between the manufacturing and service spheres of Western economies; and the demobilization of labor unions† (1997) In general, there are two views on globalization. First, is that globalization is an offspring brought by the development of new communication technology. And second, its significance and purpose lies in the linkages that it forms within states, societies and individuals (McGrew 992). The concept was hastened primarily because of the growing capability of humans to innovate technology. Communication technology played a crucial part in the globalization boom. Information technology is an extension and a means towards pushing a long term development in the technology that will aide further penetrations in the state system (Chase 1994). This has brought closer states, it made exchange of information faster and more efficient. Nonetheless, creating allies was easier. And these allies were used for trades, for economic perpetuation, for defense strengthening, and for other purpose that interdependence may serve to strong nations. While others view globalization as merely an integration among nations in such a way that it shall enable individual or entrepreneurs around the world to trade faster and cheaper, there are still scholars and activists who view otherwise. Hirst and Thomson, in their book Globalization in Question suggested that globalization is a â€Å"deliberate process of economic liberalization† that forces individuals and states to participate in a more intense market forces (2000). It is a force that pushes states to participate in order to survive. Rights and equality are merely an issue. It is a question of a state’s capability to resist the calls of globalization and afterwards suffer the consequences that may be imposed on their markets, shall they try to counter the demands of the globalizing countries. It is an issue of control over the process, and who controls the market. Globalization has reached towards the depths of a nation’s character. Ecology, culture, communication, economic and political areas were the ones that were strategically affected by globalization. This in a way, can its ideologies be pushed within a state territory. Cultural components of the human evolution has been a central target for the start of the modern world-system (Meyer 1989). The thrust in the cultural sphere has been a great start to penetrate the state system. This marked a good start into luring the economic and political aspects of a country. As culture is the heart and soul of a country, it is the first door that must be opened in order to manipulate the thinking and mindset of the target population. No wonder that the process of globalization has started within the realms of media, towards the commercial products and creating a wide array of popular culture that will penetrate the population. On the other hand, communication as mentioned above has been, and will always be an integral part in the current world system. The rapid advancement in technology has decreased the cost of communication, and this has greatly affected the communication capability. State consciousness, as a delicate factor for the formation of â€Å"global civil society† has qualitatively changed by far. Globalization of communication has altered values, culture and consciousness by means of creating or destroying something whether the state calls for it or not (Giddens 1996) The trends revealed continuities and struggles for economy and democracy based on their own reflective judgments about the emerging ideology. As Moore suggested, progressive movements must make use of the ideas emerging from economic nationalism to retract the world market forces (1995). But nowadays, the public has accepted the mainstream social, political and cultural trends that developed within and among nations as an end-result in the interactions in the expanding world system (Chase 1999). This phenomenon has yielded different opinions on the basis of its feasibility and its significance among states. When they say â€Å"economic and social progress escaping from territorial limitations and become global† – it means that society and politics has escaped the state. Everything has become global in the sense that everything has gone into the cyberspace, without borders (Hirst 2001). The effects in politics are more pronounced in such a way that state functions are no longer within borders; as such the market and internet have absorbed its functions. Another effect of globalization on politics is that, its processes have become a mere satellite heavily relying by international entities such as UN. It has become dependent with the global political forces rather than the usual local regulation (Hirst 2001). Indeed, politics have also become market-driven. The economic regulation was attributed to government and agencies that can respond more flexibly in the ever changing economic system. Murphy’s â€Å"global governance† explains the growth of international organizations. This trend of political globalization concentrated on grasping international sovereignty (1994). Alongside, this ideology has tried to undercut and argued for elimination of Second Wave institutions such as unions, socialist parties and welfare programs (Dunn 1999). Economically, globalization delocalized activities, the way it had driven away the functions of politics away from the state. These activities have been ripped from its origins and its own unique culture (Gray 1999). Globalization meant the â€Å"global-spanning economic relationships† (Chase 199). This were manifested in the growing interrelationships of markets, networks of goods and services that were institutionalized by transnational corporations. During these era, the rate of trade and investment has dramatically increased. Globalization has brought states closer by means of using higher organizations to control the wave spectrum. States look upon those entities to enforce legislations that will cover a larger market. Division of labor emerged and under this globalization picture, this shall bring order to the international community. This international order serves as ties to connect these states towards one another. Thinkers of globalization would suggest that failure to participate in this order would render them weak, for their attempt to preserve local political control will result to the demise of governance (Hirst 2001). This world system created from political globalization has lead to the emergence of hegemons, those which were in control of the trade and had the richest access to the wealthiest nations that have thick resources of raw materials that are essential to the growth of the economies of these hegemons. Gaining control of these resources, coupled with their political supremacy over the others, prompted a heavy disparity over the rights of those who are inclined to follow the ascendancy of global leaders. This is the process in which globalization has indeed grown big but left numerous states at demise. These are peripheral states that are under the rule of their core countries. And although as much as globalization had promised to provide an equal playing field for market trades, it is impossible to attain such, given that political control is always concentrated on the side of a few. This is also a reason wherein globalization hasn’t reached its outmost capability to boom. There are states that resisted this call and decided to stand outside the field of hegemonic rulers. The question on globalization’s â€Å"global† character was raised based on its function. As Hirst and Thompson argued, the system is indeed international but it is not operating truly global. It is not global because it only operates amongst national economies centered only on major states. And even as large economies participate in the global trade, most of them still keep the majority of their products towards their national economies. Migrations have also been watched after to secure the growth of their economies (Hirst 2000). Though there exists a strong connection and interdependence among countries, not the entirety of the system is globalized but fitted in an order that will allow it to compete internationally. As Castell gave employment as an example; employment has never escaped its local origin, but it has rather been a â€Å"strategically crucial economic factor that was networked for exchange of the inputs and outputs† (2001). Here, globalization has been used as a guise to import labor, for an excessive output. This is in a way increased the significance of interdependence among nations. The international division of labor has been manipulated to favor stronger nations and extract the weaker nations of its resources and capacity. Academic contention on globalization as merely a myth has risen from its characteristics that its processes have made. Globalization is challenged as just a form of â€Å"internationalization†. There are academic scholars to argue that internationalization has never progressed to globalization. Because, internalization entailed the significance of nations and role of state, while on the other hand, globalization in its full bloom state must eliminate the existence of nation states (Hirst 2000). And for one thing, the countries that have succumbed to the calls of globalization in the guise of international relations are those who were previous colonies of the fallen empires, the post-war victims, and the weak nations who had no other choice but to follow the dictates of the then superpower of America. There wasn’t a choice left but to participate in that international trade rather than being apprehended by international authorities, or be sanctioned.

Benito Cereno †Reader Response Criticism Essay

Most readers of Benito Cereno will be surprised when the African conspiracy is finally revealed. Although Melville begins the novella with ominous imagery, the text is designed to lead the reader away from the true events of the San Dominick. The point of view of Benito Cereno is the major tool Melville uses to trick the reader. The story is written in the third person, but expresses Delano’s thoughts and observations. Most readers will trust Delano’s judgement, and his observations of Don Benito and his crew set the reader up to be surprised. Firstly, Delano’s mistrust of Don Benito caused me to suspect the Spanish captain of taking part in a conspiracy against Delano. Benito asks him strange questions about The Bachelor’s Delight â€Å"with a guilty shuffle† (188). When Delano becomes nervous while watching Benito conversing privately with Babo, I was also alarmed. The image that I was most struck by was Benito Cereno standing with his face downcast, as Babo, who is kneeling down, looks upward at his master; Delano noted this contrast, and I interpreted the scene as symbolic of Cereno’s guilt. Secondly, I believed Babo to be a devoted and loyal servant. Delano was impressed by Babo’s concern for Don Benito, as was I. I never interpreted Babo’s constant service as anything but well-intentioned; in fact, all of the slaves on the ship appeared to be good-natured. Delano even reflected on the ability of the African race to mix work with pleasure after observing the â€Å"negroes† onboard performing what he thought were the orders of Cereno. He was completely unaware of the slaves’ true intentions. I, like most readers of Benito Cereno, was completely unaware of the mutiny of the slaves until Delano realized upon his departure that Don Benito had been terrified all along of Babo; however, upon rereading the text, there are many clues as to what was going on. In this new context, the image of Don Benito looking down while Babo looked up at him takes on an entirely new meaning. This clearly represents Babo’s power contrasted with Cereno’s helplessness. The same dynamic is shown when Babo is shaving Don Benito and cuts him. Delano notes that nothing â€Å"could have produced a more terrified aspect than was now presented by Don Benito† (215). During a second reading, I was also able to notice several other assertions of the Africans’ power. The two Africans who pushed aside the Spanish seaman, and the African boy who attacked the Spanish boy were demonstrating their control over the ship and its passengers. Also, when a Spanish seaman tries to answer Delano’s questions about the San Dominick’s troubles, the Africans take over: â€Å"as they became talkative, he by degrees became mute, and at length quite glum† (197). These are all important details that illustrate the underlying events of the ship, but the reader is not able to correctly interpret them during the first reading. These events are only significant to the reader once the conspiracy has been revealed. The text of Benito Cereno is designed to mislead the reader. Delano’s point of view caused me to misinterpret the events onboard the San Dominick. The typical reader is only able to uncover the hidden clues of the story once Delano himself becomes aware of the real conspiracy.

Lastly, it is hard to communicate well with someone who has a different culture or belong to a different society. The management of our own non-verbal communication is extremely important. As nonverbal communication is capable of changing the spoken words in many ways, it is important to realize that we can safeguard our own communication against misinterpretation, if our nonverbal supports our verbal message. In other words, unless we manage the nonverbal attributes of our message so that they are compatible with our words, our total message is open to misinterpretation and may be screwed up. To put it clearly, if the receiver lacks awareness of the significance of the factors influencing the nonverbal communication, then he is likely to remain confused. It can be said that a large portion of a message comes not from the words spoken, but from nonverbal attributes that can be transmitted and received consciously or unconsciously. Such nonverbal communication usually serves a variety of functions in relation to verbal communication. Examples of these functions include repeating, complementing, c ontradicting, substituting, and regulating. Thus, the person who is not only aware of the nonverbal message and the factors influencing it, but also who manages his own nonverbal communication, is less likely to be confused or confusing. Finally, it can be concluded that most researchers agree on the importance and significance of nonverbal attributes in interpersonal communication. This belief is the motive behind the quotation: "What are words when the body can bend, cry, shout, and jump! There's language in one's eyes' and cheeks, lips and hips."

These personality and belief make them stand out from all the other characters in the story. They do not have to be the lead protagonist or the main antagonist; rather, their belief or trait makes them the best in the eyes of some readers. In the case of two greatest works of fiction, â€Å"Epic of Gilgamesh† and â€Å"Othello†, it is the pride of the character and their belief in themselves that makes them stand out from the others. It is the pride and later the ability to set it aside and apologize for their deed. In the â€Å"Epic of Gilgamesh†, Gilgamesh, the great king stood out because he was the proudest of the proud. His belief in himself caused most of the misfortune that took place in the story. Only in the end did he realize his mistakes. The same may be said on Othello in the play that Shakespeare wrote. He was also proud, and this pride has caused as much misfortune in the story as did the trait of Gilgamesh. This also made him stand out from among the other characters. The Mighty Gilgamesh The Epic of Gilgamesh is an epic that originated in Iraq. It is a Sumerian legend about a great King, who is like a God. He has a strength that cannot be equaled. He is also as powerful as their gods wherein neither man nor animal can defeat him. As such, the gods decided to create another being that can equal him. When Gilgamesh met this being, whom the gods named, Enkidu, Gilgamesh immediately liked him. They easily became friends despite the initial plan of the gods to have Enkidu kill Gilgamesh. Their bond was instantaneous. They started doing a lot of adventures together, even those that are forbidden and may offend gods (Hooker). Unfortunately, in one of their adventures a god was offended and as punishment for this, Enkidu died. After Enkidu’s death Gilgamesh was almost out of his mind and did more dim-witted decisions that caused misfortunes. However, despite what he saw, he did not apologize but rather remained proud. Only after all his adventures did he come to his knees and realized his mistakes (Hooker). From this short synopsis of the epic, it may be seen that at the very beginning, Gilgamesh already showed his pride and belief in his self. He was aware of his great power and the gift of strength that destiny has gifted upon him. The king of Uruk knew that none of his subjects can defeat him or defy him. As such, he lived by this knowledge and used it to his advantage (â€Å"Epic of Gilgamesh†). He was a proud and tyrannical king and he stood out because of this. Even though his arrogant actions caused the death of his best friend Enkidu, this did not erase the fact that he was a memorable character because of it. He was much known than Enkidu not only because he carried the namesake of the epic’s title, but also because he went against the society. He did not become what other kinds are. He was cruel to his subjects. He was almost an antagonist rather than the lead protagonist due to his pride. He made the story move by not conforming to the people’s expectations (â€Å"Epic of Gilgamesh†). However, despite the seemingly terrible character of Gilgamesh he should also be commended for being proud of himself. It should be noted that to stand up and remain proud despite other people’s constant prodding to become nicer is a hard task especially for a king. Even the gods were urging him to become kinder to his subjects but he never gave in. Also, it should be noted that his pride is his strength. Because of it, he was able to overcome other difficult tasks that saved his kingdom. Had he not been that arrogant, he would not have risked his life for the tasks that he was assigned to and he would not have been able to save his people. Furthermore, he would have committed suicide as a sign of apology for his actions. Thus, with this, the non-conformity and the trait of Gilgamesh made him a character that is very memorable. The Proud Othello Shakespeare’s â€Å"Othello† is about the moor Othello, whose life suddenly made a turn because of the envy of one of the other characters. The great Othello was promoted in his job and his first action was to appoint an assistant. Iago, who was expecting to get the job, was not chosen. Instead Cassio became Othello’s lieutenant. In his fury, Iago sought revenge against Othello and Cassio (Shakespeare). In order to have his revenge realized, Iago weaved lies in the lives of both Cassio and Othello, and even in the lives of other characters involved with the two. This twisted the story to the point that this not only caused Othello and Cassio’s death, but as well as other people whose lives were connected to them. Most of the people, including Othello, believed the lies that Iago told them. By the end of the story, Othello could no longer contain his guilt and killed himself and claimed responsibility (Shakespeare). From this short synopsis it may be derived that Othello’s life had been simple in the beginning. It was only made complex by the lies that Iago weaved in order to realize his revenge. Despite the fact that Iago’s characteristics may actually make him stand out, Othello still is the most memorable character. As in the case of the Epic of Gilgamesh, Othello stood out because of his pride and belief in himself. Being a moor, many would have contradicted his promotion. The society may dislike the fact that a man foreign of nature should not lead the army of Italy. However, because of Othello’s pride and confiedence, he proved himself worthy of the title (Black). To further prove that he was not like anyone else, an instance in the story told that most men try to use potions in order to attract the beautiful Desdemona. Desdemona’s father even believed that Othello did the same when he learned that Othello was sleeping with his daughter. But then, because of Othello’s pride and self-belief, he went against the society’s expectations. He was able to lure Desdemona without potions or magic (Shakespeare). Despite the seeming good results of Othello’s arrogance though, it should not be neglected that the same arrogance killed Desdemona at the end of the story. The inability to accept the truth caused him to kill her. This may be considered saddening as it was not entirely his fault. However, he was still the one who smothered her with the pillow (Shakespeare). Othello’s pride and belief in himself made him stand out from the crowd. He did not conform to the expectations that as a foreigner he cannot be a good soldier to their country. He also proved that he can get the woman he admires with sheer manliness and not through potions or magic. Even the fact that he got himself into trouble because of his pride made his character that is not like any other. He defied society’s expectations especially when he took his life at the end of the play. This was an act of remorse for rebelling against the expectations that he did not want to apologize for. He did not conform because it would have meant hurting his enormous ego. Conclusion From the given information, it may be concluded that the two characters in the two given pieces of literature, â€Å"Epic of Gilgamesh† and â€Å"Othello†, are characters that stood out from the crowd because of their pride and self-belief. Because of being proud, they could not afford to follow what other believed them to follow. They had to show their ability to decide for their own even though it may mean misfortune. They stood out because they went past the people’s expectations and proved themselves better than any other man. They had to act according to their will because of their pride which took great strength. Positive reinforcement within the classroom Positive reinforcement within the classroom. Classroom management and discipline is the most challenging aspect of teaching (Yost Mosa, 2002). Aksoy (2003) describes classroom environment as; multifaceted, simultaneous, fast occurring, and unpredictable. This environment means that at any time, teachers have to attend to a vast range of pupil needs fairly and consistently. (Edwards 2003). It is therefore important teachers implement and refine strategies focusing on reducing behavioural issues in order to maximise potential for learning and ensure smooth classroom practise. Due to the increased diversity of pupils in schools teachers now encounter a much wider range of behavioural difficulties in an average class, this is partly as a result of the Individuals with Disabilities Education Act (Quinn et al., 2001). Interestingly the most common request for assistance from teachers relates to behaviour and classroom management (Rose Gallup 2005). Disruptions in class take up student’s valuable learning time and decreases potential learning opportunities of the class. (Witzel Mercer Miller 2003, Cains Brown, 1996) Teachers who regularly encounter problematic behaviour often report increased amounts of stress and frustration (Browers Tomic, 2000). This in some circumstances can make teachers question their capability for their role (witzel, Miller and Mercer 2003). The importance of classroom management in reinforced by Shinn, Ramsey, Walker, Stieber, Oà ¢Ã¢â€š ¬Ã… ¸Neill (1987) who found classrooms frequently experiencing behavioural disruptions had less time engaging in learning, and pupils tended to leave school with lower grades compared to peers. Within the literature it is clear that there is a wide spectrum of strategies implemented by teachers to increase effectiveness of behaviour management strategies. Some educators argue strong discipline and setting limits are most effective, others assume creating an exciting curriculum works best. All strategies highlight the importance on how to behave, pupil responsibilities and adhering to class and social norms. It is also clear that many of the strategies identified have a basis embedded in psychological principle. These include behavioural, psychodynamic, biological and developmental approaches. It has often been disputed how best to apply psychological theory to classroom practice. This review will focus on the use of positive reinforcement in classroom management. Theories will be discussed, evaluated and related to classroom practice. What defines Positive reinforcement? Positive reinforcement refers to the implementation of positive stimuli to desirable behaviour. Described by Skinner (1938) as operant conditioning, it is used to increase the probability of desired behaviour occurring again (Fontana 1994). It is most effective if implemented immediately after desired behaviour occurs and often used as a behaviour management strategy. It can be applied through social cues (a simple smile), positive feedback and reward/sanction systems and best used alongside a classroom context (rules and routines). Theoretical basis of positive reinforcement Behavioural theories have been highly influential to positive reinforcement in the classroom, although they have not always been highly regarded by the educational community (Axelrod 1997). Nevertheless the use of positive reinforcement appears frequently within literature regarding behaviour management. It is important to identify principles underlying positive reinforcement and the criticisms to these theories. Also how these core behavioural principles can be converted to classroom practise and the issues surrounding implementing techniques. The first major contribution to positive reinforcement was Edward Thorndike’s â€Å"Law effect† (1911) which implies behaviour that generates positive effects on the environment are more likely to be continued (Miltenburger, 2008). This implied reinforcement and praise play fundamental role in shaping behaviour. Shortly after this Watson (1913) describes â€Å"Behaviourism†. He bases his theory on the understanding that behaviour is learned and therefore can be unlearned. Behavioural changes are therefore due to environmental circumstances (Miltenberger 2008). Ivan Pavlov (1927) describes â€Å"Classical conditioning†, in which a stimulus is linked to a naturally occurring response that occurs with a different stimulus to evoke an unnatural response. This manifests in a classroom scenario such as using a â€Å"finger click† to attract attention. It is a strategy used to consistently hint students to stop working. This example explains a â€Å"conditioned† behaviour in which pupils have been taught to behave in a specific manor to an unrelated cue. Much of his work at the time was conducted on animals within Laboratories. B.F. Skinner’s research has been significant in the development of positive reinforcement (Labrador 2004). He argued that positive reinforcement was more effective than punishment when trying to modify behaviour. Through his study he identifies â€Å"operant conditioning† which explains how reinforcement and punishment play a key role in the recurrence of behaviour. He explains how behaviour that is consistently reinforced with praise/ rewards will occur more often. He identifies five obstacles that inhibit children’s ability to learn. These included fear of failure, complication of task, clarity, direction and lack of reinforcement (Frisoli 2008). Additionally he recognises techniques such as breaking tasks down, repeating directions and giving positive reinforcement (Frisoli 2008). Classroom research that followed was based on his findings. Studies focused on adult’s childrearing (Baumrind, 1971) and children in a Laboratory setting (Kenney Willicut, 1964; Soloman, 1964). Most studies found reward to be more effective at managing and influencing behaviour. Bandura (1965) explains these findings as a product of children’s previous conditioned responses to reward and punishment, influencing its success in class. The Importance of Positive reinforcement Positive reinforcement refers to any event that follows a behaviour used to increase the chances of the behaviour reoccurring. It can also be used to motivate students. (Domjam 2003). Bracey (1994) states; 15 years of research have confirmed that reinforcing behaviour can increase the likelihood that the behaviour will be performed under subsequent non-rewarded conditions. Similarly Miltenberger (2008) states â€Å"disruptive behaviours can be controlled or eliminated with behavioural intervention† (p11). Due to this considerable research has been conducted into the application of positive reinforcement in schools. It is used to further teaching methods, control inappropriate behaviours and improve social and functional skills (Miltenberger, 2008). Positive reinforcement techniques have however been perceived to threaten individual’s freedoms (maag 2001). Society perceives reinforcement as externally applied to an individual with the aim of coercing behaviour and leading them to become dependable on extrinsic reinforcement (maag 2001). Although an understandable concern, Akin-Little, Eckert, Lovett (2004) describe these concerns as unwarranted. Studies have found positive reinforcement to increase intrinsic motivation (Cameron, Banko, Pierce, 2001), especially rewards based on meeting a performance objective (Eisenberger, Pierce, Cameron 1999). Conroy et al (2009) describes how ‘teacher reactions’ play a significant role in influencing students behaviour, pupils will try to gain attention through predictable behaviour linked to common teacher reactions. For example, a teacher who focuses attention solely on disruptive behaviour will cause children to behave in a disruptive manor in order to gain the teachers attention. Teachers should use positive attention within feedback to influence desired behaviour (Conroy et al 2009). Chityo Wheeler (2009) highlight the importance of using positive reinforcement with pupils that show signs of behavioural difficulties. Due to the nature of their difficulties and the disruptive effect on the class it is imperative to positively reinforce desired behaviour for these students (Chityo Wheeler 2009). Positive reinforcement in the classroom. Consequences Positive reinforcement influences desired behaviour, ignoring undesired behaviour decreases the chance of it reoccurring (Conroy et al, 2009). Rules and routines are used to prevent unsuitable behaviour. They establish behavioural context for the classroom and instruct pupils on expected behaviour and the consequences if inappropriate behaviour continues (Colvin et al., 1993). This method of instructing behavioural expectations is used in nearly every school and vital to creating a productive learning environment. Chitiyo Wheeler (2009) expand on this by explaining how appropriate behaviour can be established through modelling desired behaviour and by building naturally occurring reinforcement within the classroom environment. This involves managing inappropriate behaviour through consequences (Mather Goldstien, 2001). Rules and routines play a significant role in determining a context for consequences to be applied throughout the classroom. Consequences play a fundamental role in managing classroom behaviour and creative a positive learning environment. These are highly valuable tools used to encourage learning and prevent problem behaviour (Conroy, Sutherland, snyder, al- Hendawai and Vo 2009). Consequences should be used to further learning and stop disruptive behaviour occurring (mather Goldstien 2001). Mather and Goldstien (2001) describe a consequence approach to managing behaviour. This involves defining the problem, developing a behaviour plan, identifying an effective reinforcement and constantly reinforcing desired behaviour. Reinforcement should occur soon after the desired behaviour occurs and used to teach appropriate behaviour, social and communication skills (Mather Goldstien 2001). Teachers must select consequences that are relevant and strengthen behaviour. Bushell (1973) refers to irrelevant consequences as neutral consequences that do not affect behaviour. It is important for teachers to evaluate use of reinforcements to make sure consequences are positively reinforcing behaviour of all individuals (Bushell 1973). Positive feedback Social reinforcement is significant to all children’s development and very common within the classroom. Social cues including walking around the class, smiling, complimenting or commenting are vital in increasing and maintaining positive behaviours. These can be implemented verbally; â€Å"I like the way your group is working!†, Written; â€Å"Great!† or through expressions such as clapping or nodding. Skinner (1953) suggests that pupils need significant amounts of social reinforcement and positive attention in the first few years of school. He argues it establishes several generalised social enforcers including; attention, approval, affection and submissiveness. It also promotes confidence and responsibility within children. Convoy (2009) found praise can improve the whole class environment. Effective praise increases positive behaviours and interactions with pupils and teachers. (Convoy et al 2009) Infantino Little (2005) describe a range of important principles governing praise; Praise should be initiated by the teacher, dependent upon desired behaviour, focusing on improvement, age appropriate and structured around individual needs. These different principles describe a wide range of factors that together determine the effectiveness of praise. Despite this huge range of significant factors Smith Rivera (1993) show how praise is most effective overall, applied to specific behaviour. In the literature behaviour specific praise has been linked to positive outcomes for students and a decrease in negative behaviour. (Thomas, Becker Madsen 1968, Ramsey, Walker Gresham 2004) Most importantly praise has been shown to increase student motivation (O’Leary Becker 1969) and have positive effects on tea ching reading and math’s (Gable Shores 1980). Morrison and Jones (2007) addressed the topic of Positive Peer Reporting. This positive action of saying positive things about pupils peers reduced tension, negative feelings and encouraged positivity throughout the class. An example of this in class is ‘star of the week’ or ‘show and tell time’. This shows how praise is not just teacher orientated, but in fact can be applied throughout the class in a variety of different ways. This can help promote more learning within the class, specifically areas of development etc social skills/ personality. Despite this the effectiveness of praise in diverse classrooms is questionable due to individual differences and prior experiences of praise (Lam, Yim and Ng 2008). Infantino Little (2005) noted student’s preferred to receive praise for good work privately, as they prefer not to be singled out. This may infer that in school there are underlying cultural perceptions within children that performing well is a bad thing or something to be ashamed of. Teachers can combat this by using group praise/rewards or by offering a range of rewards to individualize rewards and increase motivation. Praise is a good example of a commonly used environmental event used to reinforce student’s behavior (Brophy, 1981). Important studies suggest teachers do not praise good behavior as much as they could (Wehby, Symons, canale go 1998). Rewards systems Schools have used external rewards to manage behavior for many decades. The use of rewards for good behavior is directly related to academic and social success (Slavin 1997). On the contrary some evidence suggests that expectations of rewards can undermine intrinsic motivation (Holt 1983, Deci, Koestner, Ryan 1999). A reward is defined as; the offering of an environmental event in exchange for participation or achievement (Craighead, Kazdin Mahoney, 1981). To successfully shape problematic behavior students must comprehend and accept rewards used to reinforce behavior. Pupil’s perception of a ‘good’ reward has been shown to change with age (Shreeve, Boddington, Bernard, Brown, Clarke Dean, Elkins, Kemp, Lees, Miller, Oakley, Shiret 2000). This can have a major impact on the effectiveness of using reward systems. To combat this issue, Infantino Little (2005) state rewards should be â€Å"mutually agreed by students and teachers, realistic and deliverable†. Rewards can help define behavioral expectations, recognize positive behavior and provide a safe, consistent school environment (Shreeve et al 2002). This provides students with a sense of community and self belonging which increases opportunities for learning (Zimmerman 1989). Rewards are significant in helping motivate children with reading, Math’s and social skills (Reiss, 2005). Research suggests reward systems are effective at improving behavior (Clifton Cook, 2012). A wide range of factors contribute to the overall effectiveness of using classroom rewards. These include; perceived fairness, providing choices of reward, establishing a sense of community and individualizing reward systems. It is important that teachers evaluate their strategy to make sure these factors influence how reward strategies are implemented through school. Barriers to application of positive reinforcement. While implementing positive reinforcement within the classroom teachers face several barriers. Briesch and Chafouleas (2009) identifies that a lack of time and resources means teachers are unable to reinforce all desired behaviour within the class. Also there is a danger that the teacher becomes an unconditioned stimuli resulting in children only acting in specific ways in teacher presence (Briesch and Chafouleas 2009). Reinke, Lewis-Palmer Merrell (2008) describe ‘Locus of control’ being taken away from children when using excessive positive reinforcement, this resulting in a loss of intrinsic motivation. It is therefore crucial that teachers evaluate how productive their strategies are in their specific class. Another major issue within the literature is that when a child is rewarded for completion of a task many times, they tend to lose interest in the actual task and focus more on gaining the reward at the end. (Kohn 1993) this can seriously impact the learning environment and have adverse individualised effects on their attention processing systems (Hidi 1990). Also noted within literature is that teachers can easily make bad use of reinforcement and as a consequence, unintentionally reinforce negative behaviour. For example; sending a student outside for bad behaviour removes the child from the unwanted stimuli (work) and reinforces their behaviour because they have achieved their goal (not having to work). This means the underlying behaviour will most likely be repeated (Maag 2001). Another important fact to consider is that due to the culture of schools, and behaviour deemed appropriate, therefore reinforced, all children are being taught (reinforced) the same information decided by the small group of individuals who write the national curriculum. This does not promote individual learning and must surely have detrimental effects on the diversity and individualisation amongst the general population. Conclusion The use of positive reinforcement is based on a strong amount of literature that suggests behaviour is reinforced by contingent rewards. Most noticeable within laboratory experiments based on animals and food, positive reinforcement has become widely accepted as a behavioural modification strategy/tool. Due to the increasing range of pupil’s abilities in the average classroom teachers have to incorporate and build upon strategies that successfully promote progression with the vast majority of abilities encountered. In a class setting, strong rules and routines provide instructions to pupils that are used to direct behaviour. Verbal prompting of these rules can drastically increase the effectiveness of reinforcement. Consequences, rewards and positive feedback are used as an effective motivation tool and can supply incentives for behaving in specific ways, effectively influencing a wide range of decisions in the classroom. Praise is highly regarded by individuals and important to development of the self and social awareness. It has the ability to motivate students and build self confidence. To use reinforcement effectively through rewards, pupils must not become motivated solely by the reward, losing their intrinsic motivation can have adverse effects on individual motivation. To combat this reinforcement must be individualised for each child and teachers should incorporate a wide variety of reinforcement strategies into their classroom management strategy. It is important to note that how teachers apply positive can ultimately decide its effectiveness within the class. Strategies should be refined and built upon as children within the class develop or the teacher is at risk of reducing the effec tiveness of reinforcement and sometimes impacting negatively of their education. Positive reinforcement plays a fundamental role in creating a positive learning environment for all pupils. It is important it is used correctly and this involves reinforcing desired behaviour, not disruptive behaviour. Therefore it is important I use positive reinforcement in my own teaching by utilizing rewards, praise and sanctions to build a safe environment centred to learning. I will incorporate different reinforcement strategies into my teaching. Result: By linearization of the effective parameters of different finite element models, the relationship between effective parameters in filling phase is derived as: H=0.7456D+ 0.02174L +0.01846. This equation depicts adequate injected mound height as a function of internal diameter and intramural length, H=f(L,D). Conclusion: Using numerical simulation, we introduced the novel formula to predict the height of injected mound in endoscopic VUR treatment. As a result of this study, in order to increasing the success rate of this treatment, the ratio of mound height to intramural ureter diameter should be approximately 74% (~3/4). Current treatment options include close observation with urinary chemoprophylaxis, minimally invasive endoscopic subureteral injection of bulking agents and open or laparoscopic reimplantation[2]. The prophylactic antibiotic approach is proper for uncomplicated reflux (grade I-III without significant renal scarring or breakthrough infection)[3]. This type of treatment doesn’t cure VUR and has some problems such as patient noncompliance and increased antibiotic resistance[4]. Surgical reimplantation of the ureter is an invasive method[5] and already used for patients with high grade reflux, children with breakthrough UTI and children with reflux and developed renal scarring[6]. Endoscopic subureteral injection of bulking agents was introduced by Matouscheck as an evolution in treatment of reflux with a high success rate and minimal invasion[6], [7]. The substance used as a bulking material should be biocompatible, nonantigenic, nonmigratory and non-toxic with minimal local inflammation[8]. Multiple techniques on injection have been described. These methods include subureteral needle placement[8], intraureteral needle placement[9], [10] or some combinations of these. During recent years, several studies have been done to determine success or failure of endoscopic treatment, and various injection techniques have been introduced. Puri et al., described â€Å"volcano† appearance as the main sign of success of injection[11]. The proper shape was demonst rated by adequate coaptation of the ureteral orifice and by its location in the bladder below the ureteral orifice and/or along the waldeyer’s sheat[12], [13]. Geometry of the numerical model Fig. 1 illustrates diagrammatic representation of the functional anatomy of the ureterovesical junction and urinary anti reflux mechanism as the accumulation of urine within the bladder will lead to the tight closure of the portion of the ureter in between, thus preventing the backward return of urine to the kidneys, urinary anti reflux mechanism. In Fig. 1, P, D, H and L are intravesical pressure, intramural ureter diameter, injected mound height, and intramural ureter length, respectively. Section â€Å"A† in Figure 1 depicts the cross section of the intramural ureter with injected mound. The geometrical data of intramural ureter and ureterovesical junction were utilized as per Table 1. Materials Properties and Boundary Conditions Material properties for the intramural ureter wall were assumed to be linear elastic, isotropic, incompressible, and homogeneous with Young’s modulus of 10 Kpa [18]. The injected mound was modeled as a rigid volume as shown in Fig. 3. The boundary conditions for the wall were (1) the bottom surface of intramural wall was fixed at the connection to the internal bladder wall. (2) The upper wall was assumed to be free such that the deformation would occur due to the intravesical pressure and contribute to the tight closure of the portion of the ureter in between. Solution Method The ANSYS 11 software was employed to simulate the intramural ureter with injected mound in voiding phase. The intravesical pressure was applied on the upper portion of the intramural wall. The upper bound of intravesical pressure was considered to be 160 CmH2 O to encompass both male and female intravesical voiding pressures[15], see Table 1. The 8-Node structural solid mesh has been utilized in finite element model to have adequate adoption with this geometry and material behavior. 3 Results Fig. 2 illustrates the total deformation of the intramural ureter with injected mound in voiding phase. As shown, the intramural ureter has been closed due to the intravesical pressure. Fig. 3 shows the deformed and undeformed intramural wall in resting and voiding phase. As shown, in voiding position the injected mound height is high enough for the intramural ureter to be closed due to the intravesical pressure and prevents retrograde urine flow. In the first attempt, keeping the intramural ureter length constant, by changing intravesical pressure and internal diameter, the adequate injected mound height leading to tight closure of intramural ureter was obtained. By linearization of the effective parameters of about 30 different finite element models, the primary relationship between injected mound height, intravesical pressure and internal diameter of intramural ureter was defined as bellow, H=f(D,P) H=-0.0000003219P+0.7864D+0.000233(4) According to this relationship, it has been inferred that intravesical pressure coefficient is minute in comparison with coefficient of other effective parameters and it shows that changing the intravesical pressure plays an insignificant role in adequate injected mound height. In order to investigate the more effective indexes on injected mound height, the intramural length has been considered as one of variable parameters in finite element modeling. Varying internal diameter, intramural length and linearization of obtained data, the relationship between parameters was derived as: H=0.7456D+ 0.02174L +0.01846(5) This equation depicts optimum mound height as a function of internal diameter and intramural length, H=f(L,D). 4Discussion Endoscopic treatment of VUR was introduced more than 25 years ago and since then many different substances and injection techniques have been used with different results[13]. Choosing endoscopic treatment is reasonable, as it has many advantages, such as technical simplicity, greater acceptance of patients and parents and significant decrease in post- operation complications[7]. By improving the injection techniques the rate of endoscopic treatment has significantly increased as compared with open ureteral reimplantation[2]. During the recent years, many studies have been done to increase the ability to identify factors predicting success with the endoscopic subureteral injection[19]. At 2003, Puri et al.[11] described â€Å"volcano† appearance as the main sign of success of injection. The proper shape was demonstrated by adequate coaptation of the ureteral orifice and by its location in the bladder below the ureteral orifice and/or along the waldeyer’s sheat[12]. In different studies conducted by Lavelle et al.[12] and Yucel et al. [20], it was found that a subjectively proper mound appearance was highly predictive of injection success, but it should be noted that the morphology and location of the mound are not perfectly predictive of injection success or failure, as the mound seems perfectly adequate in some injections but the injection is not successful; also, the imperfect mound morphology does not necessarily imply injection failure[12]. Moreover, Ellworth PI et al. showed no correlation between the presence of a mound on post-injection ultrasound and the success of injection[19]. Some research agrees that an intraureteral injection technique in combination with hydrodistention results in higher success rates but this is controversial[9],[10],[20],[21]. The effect of injected volume on increasing the success rate is also controversial. Mathew D et al. showed that increasing the injection material volume will improve the success of subure teric injection[22] but other centers have shown that higher injection material volume doesn’t necessarily increase the treatment success rate[21]. So the effect of the injected volume in association with improved success rate remain unclear[12]. To determine the treatment success or failure, Tarry WF et al. described the utility of an intraoperative cystogram with a simulated voiding phase, but they demonstrated that an introperative cystogram can only detect de novo contralateral reflux but is not a proper method to predict the final success of injection[6]. Despite all the mentioned studies, still there is no definite and standard index to ascertain the technique and volume of injection, and also there is no quantitative index for determining the proper mound size and shape. So, we have conducted a study aiming to define an optimum injection mound by finite element modeling. In this study, we introduced a novel method to numerically define the relationship between some indexes in order to determine optimum mound height promise to standardize the injection technique in VUR treatment. It should be noted that due to every patient individual properties of intramural, it is not logical to consider similar injected mound height for all VUR treatment cases. Thus, it is needed to state the adequate injected mound height based on individual indexes of each case. The Intravesical pressure, intramural ureter diameter and length are some of these effective indexes. By specifying the relationship between all of these effective factors, the adequate and optimum injected mound height can be determined which can contribute to more success in treatment of VUR. In addition, it can help to save the needed injected mound height and make this injection method more economical. The results of this study show that the intravesical pressure has an insignificant effect on the required injected mound height. However, intramural length plays an important role on it in comparison with Intravesical Pressure. Moreover, the ratio of mound height to intramural ureter diameter is approximately 0.74 (~3/4), which is drawn that in order to have successful VUR treatment, it is needed that about 74% of intramural ureter diameter be filled with the injected mound. It should be mentioned that in Department of Urology of Shiraz University for the first time, an innovative method, introduced by Taheri et al.[23], is used to measure the injected mound height to assure the adequate injected mound height. In this method by adjusting camera, laparoscopy lens and imaging screen, it would be possible to measure the injected mound height as a ratio of projected picture on the screen. This study has served some limitation. The realistic cross section of intramural ureter has star shape. However, because of some limitation in finite element modeling, the circular one considered. Furthermore, we considered computational simulation only in voiding phase, although VUR may occur in resting phase. Conclusion In this study using numerical simulation, we introduced the novel formula to predict the height of injected mound in endoscopic VUR treatment. Labelling A label defines an individual as a certain kind of person. Defining an act as deviant or criminal is not a simple straight forward process. A label is not neutral, it contains an evaluation of the person to whom it is applied. It is a ‘Master Status’ in the sense that it overshadows all the other statuses possessed by the individual. If an individual is labelled as criminal, mentally ill or gay, such labels tend to override the individuals status as father, husband, worker, friend or neighbour. Whether or not the label is applied will depend on how the act is interpreted by the audience. This in turn will depend on who commits the act and where and when it was committed. Deviance is amongst other things a consequence of the response of others to a persons act. Students of deviance can not assume that they are dealing with a homogenous category. When they study people who have been labelled deviant (Howard Becker) If the agents of social control define youngsters as delinquents for breaking the law, those youngsters become deviant. They have been labelled as such by those who have the power to make labels stick. However Becker argued ‘deviance is not a quality that lies in behaviour itself but in the interaction between the person who commits an act and those who respond to it’. From this point of view, deviance is produced by a process of interaction between the potential deviant and the agents of control. Becker then examined the possible effects on an individual being labelled as deviant and that a deviant label can lead to further deviance. Citizenship, Political Liberalism and the National Curriculum I shall maintain in this essay that the civic education proposed in the new National Curriculum subject called Citizenship is not in harmony with the educational aims and principles stated in The Education Reform Act, 1988, in which the National Curriculum itself was established. I shall argue further that the present institutional arrangements for the whole of education are contrary to the spirit of the civic education outlined in Citizenship. To pursue the argument I shall draw on John Rawls’ insight that, in a modern democracy such as that in the UK, the idea of a democratic state with a single generally agreed moral or religious doctrine is no longer useful. In Political Liberalism he writes about this notion: That conception of social unity is excluded by the fact of reasonable pluralism; it is no longer a political possibility for those who accept the constraints of liberty and toleration of democratic institutions. (p.201) He uses the notion of justice as fairness to indicate how the state may deal justly with its citizens in a pluralist society. He defines justice as fairness in terms of two principles of justice. The first is that all people must have the same political rights and liberties. The second principle is that of equality of opportunity. Rawls then restricts the sphere of influence of these principles to that of political, social and economic institutions. This position he calls ‘political liberalism’. He maintains that this political liberalism should be seen as a freestanding moral system applicable only to political, social and economic institutions. This is somewhat difficult to swallow if only in terms of exactly how this limit... ...ply to all pupils. Pupils do not receive the same civic education. Fourth, the institutions are not freely available in terms of fair competition although both systems are state controlled.
